2011 Worker Safety and Health Violations at Savannah River Site

The Department of Energy (DOE) issued a Preliminary Notice of Violation (PNOV) to Savannah River Nuclear Solutions for violations of DOE worker safety and health regulations. The violations involved a July 2011 incident in which a worker using a scaffold to perform facility modifications in the Purification Area Vault at the Savannah River Site fell and suffered injuries to his head and torso. The PNOV cited four violations in the areas of: (1) identification, assessment, prevention, and abatement of hazards; (2) scaffold safety; (3) training and information; and (4) occupational medicine. DOE proposed an adjusted civil penalty of $159,375 for the violations.
