Federal Investigation of Torrance Refinery Explosion

A U.S. Chemical Safety Board (CSB) investigation into a February 2015 explosion at the ExxonMobil refinery in Torrance, California found multiple weaknesses in the refinery’s process safety management system. (Exxon sold the refinery in July 2016.) The CSB found the refinery did not perform a sufficient hazard analysis, used insufficient safeguards, operated unit equipment beyond its predicted safe operating life, and lacked safety instrumentation to detect flammable hydrocarbons.
